FORMS

In this section, you will be able to download most of the forms you'll need during the season.

Most forms may be downloaded in the following two formats

  • Microsoft Word (.doc), displayed by the blue "W" icon -
  • Adobe Acrobat (.pdf), displayed by the Adobe "pdf" icon --

    • You will need one of these two programs on your computer to download these forms. Microsoft Word is a word processing program that is available for purchase from Microsoft. Adobe Acrobat has a program, Acrobat Reader, which allows you to view and print any document -- regardless of what fonts are used. You may DOWNLOAD THIS PROGRAM FOR FREE from Adobe.

    Many of the forms are Interactive, thanks to fellow Metro Atlanta Umpire Dana Mason. Those forms will have two stars next to them (**). Interactive forms will allow you to enter information directly to the form. Then you may print or e-mail the form. They look very professional.

    The best way to use these forms is to download them directly to your hard drive. You may do this by right-clicking on the icon-link of the form you want, then choose "Save target As..." from the menu. A pop-up window should appear asking you to choose where on your computer you would like to download the form. You may also rename the form if you wish. The other option is to simply click on the icon-link of the form you want. It will open in a new browser window.
